Protocols for COVID-19 Symptoms, Exposure, Illness, and Travel

HPMS Safety Protocols are outlined in our COVID-19 Guidance Document for the 2022-23 School Year above.

Please note that should we need to revise any of these protocols as the situation and/or governing authority guidance evolves (HPMS follows guidelines issued by JC Department of Health and NJ Governor Executive Orders where local guidance supersedes CDC), HPMS will send all updates via email.

All of these forms are also included within each relevant section below but here are the links for your convenience:

Symptoms/Exposure to COVID

When an HPMS community member experiences COVID symptoms or has been exposed to an individual that is COVID positive, they must complete the 3 steps below before their return to school:

  1. Take a Covid test and email test results as soon as possible (At-Home tests are permitted for ages 2+)

Any individual exposed to COVID or with symptoms including but not limited to runny nose, sore throat, upset stomach, headache, cough, nasal congestion, conjunctivitis, rash, and loss of taste and/or smell must take COVID test and immediately send a labeled photo with classroom name, date of birth, and child’s name or copy of the  results to Covid@HPMSJC.org  and Nurse@HPMSJC.org. This enables HPMS to contact any exposed individuals in a timely manner.

2. Complete the Symptoms/Exposure Form

3. Receive Clearance Email from Covid@HPMSJC.org

For clearance to return to school, an individual must  be symptom-free and medication free for at least 24 hours.

Please inform HPMS that the student has been either (1) symptom-free and medication free for at least 24 hours or (2) in the case of exposure, state the last date of exposure and if the COVID-positive individual is part of the household and email Covid@HPMSJC.org  and Nurse@HPMSJC.org.

Reporting COVID Test Results and what to do after hours and on weekends:

  • All negative COVID tests should always be sent to Covid@HPMSJC.org with a copy to Nurse@HPMSJC.org

  • Reporting information to us after hours or on weekends: Use this cell phone number (347) 979-7510 to text any news of a positive COVID test only if this testing information becomes available after 4pm or on weekends. This will enable us to respond faster when necessary. This cell phone number is only to report a positive Covid test for your student.

Please continue to email Covid@HPMSJC.org, Nurse@HPMSJC.org, (with a copy to your Classroom Teachers) during the regular school hours for symptoms, travel, and questions regarding COVID.

Covid Vaccine Card Info

Please keep HPMS aware of your student’s Covid Vaccine information including additional doses and boosters to ensure HPMS is aware of current vaccination status in the case of possible exposure and travel.

TRAVEL

Unvaccinated International Travelers must complete this form for clearance to return to school.
